Rocky Mountain Pace

Program of All-Inclusive Care for the Elderly (PACE) is operated by Health First Colorado (Colorado's Medicaid Program) and Medicare. The PACE program provides comprehensive medical and social services to certain frail individuals 55 years of age and older. The goal of PACE is to help individuals live and stay in their homes and communities through comprehensive care coordination.

Developmental Disabilities Health Center (DDHC) focuses on providing exceptional health care for adult patients (18 and older) who have a Developmental Disability. This includes individuals with impairment in physical, learning, language or behavioral areas. Developmental Disabilities include, but are not limited to, Angelman Syndrome, Autism, Cerebral Palsy, Down Syndrome, Fetal-alcohol Syndrome (FAS), Fragile X, Freeman Sheldon Syndrome and other cognitive and intellectual disabilities.

Knob Hill Urban

Knob Hill Urban Arts District is a grassroots initiative to improve the Knob Hill business district of Colorado Springs by creating the city’s first neighborhood centerpiece for street art. In 2017, Knob Hill business owner and street artist Paes164 organized other local businesses along Platte Avenue between Union Blvd. and Circle Dr. to begin creating the Urban Arts District together. Over a dozen large outdoor walls were pledged for murals to date, with several large scale works finished or in process. More inspiring projects are planned.

Project Angel Heart

Home-Delivered Meals Project Angel Heart was founded in 1991 to address a major challenge for Coloradans who are ill: getting the nutrition they need to get stronger, heal, and remain at home. Many are too sick to get to the grocery store or unable to cook for themselves. Others find they have to choose whether to buy food or medication to make ends meet.

Silver Key

Silver Key Connections Café provides hot, nutritious meals in a social setting to eligible diners, promoting good health and offering the opportunity to enjoy friendship with others. Connections Café meal sites are located throughout the community and reservations are required. Diners receive nutritional information and education as well. Funding for this service is partially provided by the Pikes Peak Area Council of Governments/Area Agency on Aging.

Fountain Valley Senior Center

We provide services to the Seniors of El Paso County, including transportation, lunch, frozen and shelf-stable meals, classes and socialization.

Teller County Senior Coalition

The Teller Senior Coalition is here to help Teller County Seniors. We provide Transportation, Case Management, Caregiver Support/Respite Care, Homemaker Services, Home Delivered Meals and Safety Related Home Improvements. We also run a Public Shuttle on Wednesdays and Fridays in Woodland Park.

Peak Nutrition Clinic

UCCS Aging Center Peak Nutrition Clinic helps individuals with a combination of personal counseling, workshops, and team services.
